As a Transitions Link Worker, you will work closely with young people with complex and special educational needs, aged up to 25. Helping those with the offer of support and enablement to help them lead independent and enriched lives. Working for Swindon Borough Council, you will focus on young people in the Swindon community, those who have received social care help with complex needs and learning disabilities and intervention, therefore as young adults, they will receive the required levels of support to include employment, social activities, personal care and full independent living.

The role of Transitions Link Worker is a permanent post, which offers an attractive salary and enhanced employee benefits. You will have access to wider learning and education in order to acquire new skills and help with career development. Based on full time hours Monday – Friday, the role will involve travel to and from different locations.

Working with young people on a 1-2-1 basis, you will manage a number of individuals, taking valuable time to ‘get to know them’. Understanding their individual needs, whilst working with Social Workers, Care Managers and other professional bodies to build the very best life plans in order to ensure positive outcomes for each and every young person.

Working closely with Schools and other Educational establishments to develop individual educational plans, offering choice and individuality in the outcomes of all concerned. You will have the ability to build excellent relationships based on trust and knowledge, able to communicate between the individual, the families and all partnerships involved in the service.  You will have excellent working knowledge of Outlook, Word and Excel for the use of report writing, email correspondence and the ability to produce spreadsheets for capturing expenditure and outcomes.

Ideally your career to-date will evidence previous working with young people, families and carers, with experience of Health, Education and Social Care.
