Team Manager


The Adult Social Care Team at Swindon Borough Council is looking to recruit 2 x  permanent Team Managers who will be based in the Great Western Hospital (GWH) Social Care Team). You will be responsible for leading a team of social care professionals who offer a 7 day hospital discharge service (including weekends). 

These permanent vacancies are highly challenging and ever-changing, therefore the need to organise, plan and deliver best outcomes will need to be managed effectively and efficiently. The reason is that the 2 x Teams Managers will manage 4 individual teams within this service area;

  • Team 1 and Team 2 - Social Care Team’s that support National Discharge Policy from GWH and Swindon Intermediate Care Centre.
  • Team 3 – Social Care Team that facilitates ongoing Multidisciplinary assessments for people in our Discharge to Assess and Reablement/Pre Assessment Services.
  • Team 4 – Social Care  Team that facilitates Rapid Social Care assessment and services for those in need of urgent, same day response, to avoid a hospital admission.

As a Team Manager you will ensure that the teams provide a high quality service to Adults with needs including older people, those with physical disabilities and patients being discharged from hospital in line with Care Act 2014 (CA 2014). Responsible for ensuring that the remains focussed on good outcomes with timely interventions at the right level all within statutory guidelines and within given resource and budget availability.  Consistently working with the Service Managers on matters of operational development, trends in the usage of services and unmet needs will form key elements of the role.

You will ensure the appropriate legal framework for Councils with Social Care Responsibility is applied lawfully, effectively and affordably.  Ensuring the organisation fulfils its responsibilities within the statutory and local performance frameworks and in line with national and local policies. Effective and correct delivery in the safeguarding of adults principles, The Mental Capacity Act and The Human Rights Act are fully considered throughout each and every action.

Naturally as a Teams Manager you would expect to have extensive communication, both verbal and written with a number of professional bodies, to include Medical, Health, Commissioned Services, Emergency Services, family members and extended networks.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service. Satisfactory references will be required along with educational evidence of HCPC registration (Health and Care Professions Council) and a recognised social work qualification. 

As part of the COVID19 strategy this role will be subject to regular covid19 testing, as per the guidance set out by the Department of Health and Social Care. Applicants will need to evidence double COVID vaccinations due recent Government regulations.
