About the role
You will work within our Ongoing Support Team to offer social work services to those who require longer term social care support.
We help those most in need by listening and responding to their specific needs, and work with people to identify what is important to them and support them to make a plan to improve things that aren’t going so well, so that they can live the life they want.
We work with health partners to support people before they reach crisis point. We focus on identifying existing individual, family, and community support, ensuring that people can live independently for as long as possible.
Rather than assuming responsibility for providing care, we look for local, sustainable, and less intrusive solutions wherever they are available. We help people access the wide range of opportunities for intervention and prevention that are provided.

If you are newly qualified, we will support you to complete your ASYE year.
You will need to be registered with Social Work England and should have well-developed interpersonal skills and the ability to effectively communicate with people in a variety of ways and at all levels.
In addition, you will have an up-to-date knowledge and understanding of legislation and guidance related to working with, and the safeguarding of vulnerable adults.
Knowledge and understanding of Health & Social Care legislation and wider policy context, is also essential.
