About the role
As a Social Care Practitioner, you will be part of a newly established service, supporting young adults to fulfil their potential. Initially supporting young adults with learning disabilities, we hope to expand our remit over the next two years to support all those aged 18-25 with eligible Care Act needs.
Within the service, you will work closely with SEND, children’s and adult’s social care, and other organisations to ensure that individuals who are growing up, move seamlessly between children’s and adult care services.
You will have excellent communication and negotiation skills as well as the ability to prioritise tasks and work on you own initiative to meet deadlines.
