We’re seeking enthusiastic and motivated Social Care Practitioners to join our dynamic and supportive Locality Teams (Discharge Services).
Team members are currently working from home with a small amount of daily on-site presence at the acute hospitals (Royal United Hospital, Bath; Great Western Hospital, Swindon; Salisbury District Hospital).
About the role
The role of Social Care Practitioner is essential to supporting customers in our busy Discharge to Assess and Intensive Rehabilitation services. Your main duty will be to carry out Care Act Assessments with customers and their carers, working with them to arrange support plans for those who have eligible needs for long-term care services.
Where possible we try to support people to return home, but some customers will have significant care needs, complex social situations or rehabilitation needs that mean they have not been able to return home safely from the hospital. As the Discharge to Assess and Intensive Rehabilitation Services are short-term, assessments, support plans must be carried out as quickly as possible.
The service works in collaboration with Wiltshire’s hospitals, primary care, and Wiltshire Health and Care services, as well as organisations such as Carer Support, Age UK, and Appello (telecare).
The Locality Teams consist of Social Care Practitioners, Social Workers, Advanced Practitioners, Assistant Team Managers, and Team Managers.
Social Care Practitioners can expect to receive monthly supervision from a Social Worker and daily/as needed access to peer and leadership support and advice.
You must be customer-centred, take a strengths-based approach within your work, and have experience of working in a health or social care setting. Excellent verbal and written communication skills, and IT skills are also essential to this position.
This is a fast-paced role, so you must be able to work collaboratively with multi-professional teams and take responsibility for your day-to-day caseload management.
