An exciting opportunity has arisen for six Social Care Practitioners to join our innovative and forward thinking Reablement Service.

Fixed term - 2 years.


Our service is based in our three main hubs, Trowbridge, Salisbury, and Chippenham. We offer an occupational therapy led service to our customers which is delivered by our in-house support teams. We work with individuals and communities to promote resilience, independence, and wellbeing.

About the role

As a Social Care Practitioner, you will provide person centred and outcome focused assessments to identify the support required to enable customers to make positive changes to their lives.

Customers will have recently been discharged from Hospital and will be receiving short term support from either our Reablement or Community Health Homefirst Team, your role will be to assess and identify their long-term support needs in line with the Care Act 2014.

You will promote customer independence, choice, and well-being in the community, using universal services and support where appropriate.

This is a challenging role, and you will be supported by experienced Occupational Therapists, and a highly experienced Management Team.

About you

You should have knowledge and experience of working in a Health and Social Care setting, be motivated to work towards a relevant qualification.

The work is fast-paced, and you should have excellent communication skills, and the ability to work collaboratively with multi professional teams.

Good IT skills are essential, and experience of Liquid Logic is beneficial but not essential as on the job training will be provided.

Where possible interviews are virtual, but due to the nature of this role interviews will be in person. Current public health advice is followed for candidate and employee safety.
