Social Care Housing Lead

As a conscientious individual, you will have exceptional capability to communicate with others by using your inter-personal skills. These skills will be utilised through interviewing and assessing applicants and tenants in their own homes plus office-based interviews, whilst presenting to a range of people at senior level. You will be requested to use your proven communication skills during meetings, liaising and building relationships with all social care, health and other professional teams.
When working as a Social Care Housing Lead, you will act as the main point of contact for all Social Care colleagues, facilitating meetings with Social Care and Housing Providers to establish priority and suitability of applications for vulnerable adults in supported accommodation, those in Extra Care, in adapted homes, pathway accommodation and in sheltered housing. The successful candidate will be responsible for ensuring cases required for hospital discharge are resolved by identifying suitable social housing or referral to Extra Care; enabling people to move on from temporary accommodation as quickly as possible.
Within Swindon’s Housing Allocations policy, you will assist with the overall objectives for the team and for the wider council and will require a good understanding of the main legislative framework within Housing and Adult Social Care. You will be able to demonstrate resilience and the ability to prioritise and manage competing demands, extracting and interpreting information to make decisions within set guidelines without supervision. The successful candidate will require strong organisational skills to succeed in this role
With excellent administrative skills you will be highly competent in using IT packages, including databases, spreadsheets and word processing. You will have experience of writing reports, highlighting key areas of information and carrying out audits on specific service areas. You will have had experience of designing and delivering effective training and/or learning interventions to other colleagues or professional partners.
Previous experience of working within social housing and/or in an Adult Social Care environment dealing with vulnerable adults is key. We require someone who is a team player, has strong organisation skills and the right behaviours. You should have a minimum of grade C or Grade 4 in Maths and English at GCSE level or equivalent. Level 3 Housing and/or Social Care qualification is desirable but not essential for this post. Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
Experience of lone working is required and you will hold a current valid driving licence and have the ability to travel throughout the Borough of Swindon. An enhanced DBS disclosure will be required.
