We have a rewarding opportunity for you to join a brand-new team that are part of Home Group's new Transforming Care agenda. Your team will work with our customer (let's call her S) who has complex needs, including a moderate learning disability, autism, communication challenges, ADHD, some sensory processing challenges as well as a diagnosis of psychosis.
S, who's in her late teens, currently lives within an institutionalised environment. You'll help her adapt, transition, and settle into living in her first ever own home. You'll soon discover that S is an intelligent, independent, chatty, super direct and complex young lady who enjoys her imaginary friends! S needs a dedicated and positive team to help her reach her full potential.
S has some sensory processing difficulties and can react strongly to touch, both in positive and negative ways, as well as being sensitive to strongly smelling toiletries. When stressed or anxious, S may challenge us, and we’ll make sure you are trained to a high standard to manage on those occasions should they occur.
S loves spending time doing the things she enjoys and that are important to her; she adores her cuddly toys, watching Netflix, swimming, horse riding or simply soaking in the bath. Unfortunately, being outside can be a challenge for S when things are really busy and there are lots of people around, so we need your skill and expertise to help open up her world when she moves into her new home.
As part of our team you'll help S overcome her barriers to help her lead a fulfilling and meaningful life and reach her true potential. You'll recognise her strengths and abilities and help her take the small steps at her pace, never losing sight of her dreams and aspirations! You're patient and calm, able to adapt your style to S's needs, as well as to the specific occasion and circumstances, even when she challenges us.
S needs you to have a gentle nurturing and caring approach to help her settle into her new home and new life and be proud of how you can help her grow. With the help of our Team Leaders and clinical team, you’ll help S to participate in her favourite things as well as try new things through positive risk taking. You’ll work alongside colleagues providing a consistent approach when S is finding things difficult as well as finding creative ways to help her participate in daily living activities to increase her independence.
About our team
You’ll work for Deb our Registered Manager who has worked for Home Group for 5 years. Deb has loads of experience in working with customers with both mental health and learning disabilities and has a clinical background in nursing. Deb was also an accredited psychotherapist in a previous life before joining Home Group and is perfectly placed to help you settle into your new role! Outside of work you’ll find her shouting for her local football team, spending quality time with her family and Deb just loves cooking for friends.
Where you’ll work
You’ll work primarily with S in her new home in Swindon, as well as helping her out and about in the local community. During the transition period when we prepare to move S from her current accommodation in the Midlands, it will be important that we all get to know each other. To help us do that, there may be some in-reach which will take place electronically and you may need to travel to meet with her face to face, although we will try to limit that requirement as much as possible. It would be awesome if you drive and had access to your own transport to get from a-b as public transport in Wiltshire can be a bit of a challenge!
Working hours for this job
We will be with S 24/7 to meet her needs, so there will be a variety of working patterns available. Ideally, you’ll take your turn with your team working on a rota which could include working days, evenings, weekends, bank holidays and waking nights. You may also be required to do sleep ins or be on call where we apply an enhancement. Our days team members will work with S between 7am and 10pm, and our night workers between 9pm and 8am.
If you are not able to do all shift times, we still want to hear from you, we are building a brilliant team from scratch and we want to be flexible with our approach. As we’re asking you to be flexible for our customer, we’ll also give you the time back that you need to spend time doing the things that matter to you!
You’re passionate and motivated to assist S with any aspect of her day to day living. You’ll assist S to enjoy the best possible quality of life. You’ll help her change her life for the better by involving her and her network in working towards her aspirations. You’ll provide prompts to encourage her own personal care, as well as use creative solutions, and assistive technology where available to aid her independence.
You've worked in a similar role before and have experience of helping customers to reach their full potential. Holding level 2 Diploma in Adult Health & Social Care or equivalent, you have a great insight and passion for promoting independence and social inclusion through person centred approach. To be brilliant you will have previous experience in helping people using PBS.
When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one.
In line with current Government legislation for CQC services, you must be fully COVID-19 vaccinated to work in this service, unless you are exempt. A job offer will be subject to providing evidence of this condition of employment.

If required (and only as a last resort), you may need to use physical intervention and we will provide you with training, help and knowhow to do there safely.
