Live Well Response Worker
Hours: Full-time and part-time contract type: Occasional evening and weekend work as required.
Your role will be about providing practical and emotional support to adults living in Swindon, who have been identified as needing additional support. You will assist them to regain and maintain skills in order to maximize their resilience and independence, allowing them to enjoy the best quality of life.
As a Live Well Response Worker, you will be responsible for managing a caseload and will work closely with colleagues to ensure the most suitable support is in place and reviewed accordingly. An element of the role may require manual handling to support the facilitation of hospital discharges which you will receive the suitable training. You will receive ongoing supervision and training as well as peer support from team members.
you will have excellent communication skills and be confident working on a one-to-one basis with vulnerable individuals with a qualification of least level 2 in Health and Social Care or equivalent.
This role has been created as part of a new and exciting collaboration between Adult Social Care and Public Health. The role is permanent and part of an ongoing partnership to ensure people are receiving the right level of support at the right time.
