Level 2 social worker - Rapid Response Team

We are looking for a Level 2 Social Worker to join our Rapid Response Hospital Social Care team in Wiltshire.

This is an exciting opportunity for you to work closely with Social Care colleagues and a developing multi-agency integrated service combining Health and Social Care services.

The aim of this role is to reduce unnecessary hospital admissions/carer breakdown and to promote independence, by assessing people in their own homes and assisting people to maximise independent living.

As a Level 2 Social Worker, you will assist your manager in providing a comprehensive integrated service, safeguarding vulnerable adults, and promoting rapid support and care for adults and their carers during a crisis, for example a fall or carer crisis.

We are developing our work in supporting people in the community prior to conveyance to hospital, but also support Accident and Emergency departments. You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being.

You will keep the individual at the centre of all activity and address assessed needs within the legislative framework.

About You

We’re looking for a highly motivated and organised registered Social Worker who can bring energy, flexibility, and creative assessment/care planning to the role, for the individual and their carers by embracing informal support networks, community engagement, and partnership working.

You should have a good understanding of the relevant adult social care legislation and the ability to work autonomously within agreed timeframes and manage frequently changing circumstances.

You should be able to work as part of an integrated team and have excellent communication skills.

This post receives a 5% market supplement payment which is reviewed annually.
