We have an exciting opportunity for Qualified Level 2 Social Workers to join our Ongoing Support Team in Chippenham.
2 x Permanent - Full Time
1 x Permanent - Part Time (35 hours per week)
Our Ongoing Support Teams offer social work and occupational therapy services to those who require longer term social care support.
We help those most in need by listening and responding to the specific needs of the individual; working with them to identify what is important to them and supporting them to create an improvement plan so that they can live the life they want to.
We work with health partners to support people before they reach crisis point. We focus on identifying existing individual, family and community support, ensuring that people can live independently for as long as possible.
Rather than assuming responsibility for providing care, we look for local, sustainable and less intrusive solutions wherever they are available. We help people to access a wide range of opportunities for intervention and prevention.
About the role
You will be part of a supportive, evolving, and multi-disciplinary team, developing a service that puts the customer at the centre of everything we do.

You must be qualified to a DipSW Level or equivalent and be registered as a Social Worker with Social Work England.
