This is a supportive and knowledgeable team that has had to adapt to changes in working in response to the coronavirus pandemic.

The role is open to new social work graduates looking to start their ASYE year, or qualified social workers still gaining formative experience.

About the role

The team provides advice, assessment, and discharge planning for customers who live in Wiltshire. We work closely with health colleagues to promote an effective multi-disciplinary approach. We mainly work on a 'Discharge to Assess' model where customers either return home or move to care home placement from hospital before having their needs assessed. We also work closely with partner organisations to support decisions about hospital discharge pathways.

As a Social Worker, you will be expected to hold a caseload and undertake Care Act Assessments as well as assessments of mental capacity with customers, to facilitate discharge from hospital and Discharge to Assess services.  As you progress, you will take on additional responsibilities, such as investigating officer work in response to safeguarding alerts, or supervision of social care practitioners within the team.

About the team

The team comprises a leadership team of a Team Manager, Assistant Team Manager, and Advanced Practitioner Social Worker. We have Experienced Social Workers, Social Workers, and Social Care Practitioners on the team and access to administrative support.

The team has a good track record of supporting newly qualified social workers through early years in the profession; we are proud that people who started in this team under ASYE over the years are now in Experienced Social Worker and Advanced Practitioner posts within the adult care services in Wiltshire.

This is what our most recent newly qualified social worker has said about joining the team: “When I joined the team I was very nervous to start somewhere new, my last placement at university was quite challenging so on joining this team I was very surprised at the support I received straight away. I had a thorough induction and plenty of shadowing opportunities which really allowed me to understand the work that the team does, as well as introduce me to everyone. No two cases will be the same, and this has allowed me to develop my knowledge in different aspects of the work we do, whilst receiving support through supervision. I have spent time working in the hospital and different care homes in the area, which has provided me the opportunity to work with a range of different people. I have really settled into my first job since completing university and I cannot thank the team enough for their support over the past few months.”

About you

You will be registered with Social Work England and able to demonstrate experience of working in adult care settings. Knowledge of the Mental Capacity Act 2005 and Care Act 2014 is essential and ideally, you will have experience in applying these legislative frameworks.

You will be well organised, self-motivated and proficient with IT systems. There are office bases at the Council offices in Salisbury (Bourne Hill) and at Salisbury District Hospital. You will have the option to work from home for administrative work.
