Hospital Discharge Planning Assistant required to provide a service for social care referrals received from The Great Western Hospital in Swindon. This is a permanent job opportunity, offering full time hours working Monday to Friday, working for Swindon Borough Council. Working as part of the wider discharge planning team, you will be working in a fast paced working environment, where communication is key, aiming to reduce the delayed transfer of care for those who need to leave hospital after receiving care and treatment.
As a Hospital Discharge Planning Assistant you will have exceptional communication skills, both verbal and written with the added knowledge of working with teams meetings, email correspondence and arranging physical meetings between a number of professional bodies. Responsible for gathering sensitive and personal information from patient records, hospital and care key workers, ward staff and care providers all play a hugely important role in providing the very best outcomes for those needing to leave hospital and returning home or transfer to a residential care facility.
Ideally you will have previous work experience gained in a busy administration/communication role, where you have had to take responsibility for a number of communication methods, providing solutions to issues and supporting busy workloads. Ideally educated to GCSE Level in English and Maths with excellent working knowledge of Outlook, Word, Excel and SharePoint would be highly advantageous. No one day is the same, as each and every person’s needs are different, therefore the need to work in an organised and methodical way is important, taking time to read all notes, record and save data, to communicate with all parties and bring about the best solutions for those involved.
When working as a Hospital Discharge Planning Assistant you will receive all necessary training in Health and Safety, The Care Act and Health and Social Care.
