This role really focuses on the social, emotional and practical life skills needed by each and every one, consistently working with Adults who simply need help to secure happy, healthy and stimulating lives.
The success of the Community Support Worker depends on your core values and skills, your naturally sociable personality and the ability to listen, have patience and support people with life choices. Working with individual care plans, supported by other health and social care professionals, families plus wider social circles will help form the key objectives to a successful outcome for all involved.
You may be involved with the hospital discharge program, ensuring that if medical assistance is required, you will help to organise and facilitate for all concerned. Maintaining accurate records and providing necessary updates for the wider management team all form key tasks in this busy and challenging role.
The role of Community Support Worker will entail working a flexible shift pattern, days, evenings and some weekends, as after all help and support is a 24/7 requirement. You will need your own transport in order to travel around the Swindon area.
