Care Planning Assistant Manager required for immediate start, based here in Swindon and working full time hours Monday to Friday. As a Care Planning Assistant Manager you will work as part of the wider Social Care Team at Swindon Borough Council, responsible for carrying out assessments with older people and those who have a physical disability, identifying areas of need. You will also assess any risks to ensure that the correct services are applied all in line with the delivery of ‘person centred care’.
This is a truly busy, rewarding and demanding role, which does require previous Social Work employment experience and knowledge, best suited to a candidate who has a good understanding of The Care Act 2014. You may have worked in and around a number of Social Work settings, however this career opportunity does provide a development platform for someone who is looking for a Care-Planning role. You will have many duties, which will include providing advice, obtaining and sharing relevant information to sign post other services and managing a your own caseload.
For the role of Care Planning Assistant Manager you will have excellent written and verbal communication skills, the ability to complete complex and sensitive data, liaising with finance, carers, family members and other professional contacts. The need to ‘bring everyone together’ is vital to the overall success of the care plans provided, therefore the ability to communicate via teams, email and in person all play a crucial role in this position.
You will attend team meetings with your work colleagues, 1-2-1 service user meetings and wider meetings with a number of professional bodies (GP’s, Hospitals, Housing, Education and Mental Health), all with the aim of providing plenty of opportunity for discussion, to share possible solutions and to deliver the required outcomes. Ideally you will have NVQ Level 3 in Care (or similar) and be willing to undertake further training and education in line with the service requirements.
